19.3 - 504 Plans
A 504 Plan is developed to ensure a child who has a documented impairment that substantially limits one or more major life activities receives accommodations that will ensure their academic success and equal access to the learning environment.

Returning student with an active 504 Plan | Must have an Annual 504 Review | The Student Support Advisor assigned will reach out to the team to schedule this meeting by the Annual Review date. |
Newly enrolled student with an active 504 Plan | Must have both a meeting upon enrollment and an Annual 504 Review | The Student Support Team monitors new enrollment for parent reports of an active 504 Plan and the Student Support Advisor assigned will reach out to schedule the new enrollment meeting as soon as possible. An Annual 504 Review will also need to be conducted by the due date. |
New 504 Plan Referrals |
New 504 requests | A 504 Eligibility Evaluation will be conducted and 504 Plan developed if the student is found to be eligible | The HST should complete the survey below to request an initial eligibility evaluation. The Student Support Team monitors this survey regularly. The Student Support Advisor assigned will reach out to the team to begin the eligibility process. |

Other Important Information
- As a 504 Plan is a legal document, the Student Support Advisor will send a final draft to the team, including the HST, for signature after the meeting has been held and the document has been finalized. A new 504 Plan will not be active or able to be implemented until signed consent from the parent has been received.
- A 504 Plan does not preclude a student from requiring additional support. All students with an active 504 Plan should receive Tier 1 MTSS support as needed. In addition, should further supports and interventions be required, Tier 2 support may be requested and/or an SST meeting for Tier 3 supports may be held to determine how to best meet the student's needs.
- A 504 Plan is not the same as an IEP. Typically students either have one or the other. For further information, click this link: 504 Plans vs. IEPs
