Please use the following google form to submit a request for an initial assessment for special education. You will be able to upload documents related to the request as well.

Requests for Initial SPED Assessment

This survey will alert the Student Support Team that an SST meeting is required IMMEDIATELY in order to respond to this request. It is critically important that the HST work with the Student Support Advisor to be available to meet, as well as to encourage the family to work with the team.
