Educational Access Referral Form

Please fill out the above survey if you have a PCA student experiencing personal/situational hardships or challenges requiring short-term educational programming changes, reduced schedule request, medical leave requests, educational continuity, and/or support and coordination from the Educational Access Advisor and Student Affairs Department. These challenges/concerns may include but are not limited to:

  • A student/family experiencing a traumatic event such as an assault or accident
  • A student/family experiencing significant loss such as the death of a loved one or bereavement
  • A student/family experiencing hospitalization or a serious illness or injury
  • A student/family being placed in a facility (5150 or Correctional)
  • A student who runs away from home or goes missing

If there are multiple students in the same household who are also impacted by the same concern, please complete only one form for the family.

Other Concerns

If you are seeking mental health support for your student, please review the Mental Health page of the HTH.

If your student/family is suddenly experiencing homelessness, please ensure you have already have reviewed HTH 20.3 and notified Yolanda Osborne, Director of Special Programs of the student's circumstances and started TK-8 Tier 1 Academic Support.
